Our Mission is to ease the decision making process for hospital patients and families during their transition toward recovery. This is achieved by providing participating hospitals with an interactive search tool of post hospital care providers that is quick, simple, and tailored to meet the patient’s needs.

Our Story

The Interactive Care Network (ICN) is comprised of health care professionals with more than 75 years of combined experience in the nursing and rehabilitation industry. We have personally experienced, as well as, witnessed countless people faced with the uncertainty of health care options beyond the hospital setting. We also recognized a disconnect between hospitals and the community healthcare providers in larger city markets that created gaps in the information that was being provided during the decision making process.

We created ICN to bridge the information gap between community provider, hospital, and patient so that important health care decisions can be well informed and collaborative.

A critical step to engaging this type of communication during the discharge process was making sure patients and family members could actively participate while they were in the hospital. ICN accomplished this by developing partnerships with interactive patient communication providers. Our partners quickly saw the value in providing this information for their hospitals and patients which positioned ICN to be a truly innovative, unique and effective medium for providing information.
